Legacy Healthcare Services, Inc. is seeking a Rehab Director (PT or PTA) to lead rehabilitation efforts in our facility and serve as the primary Legacy representative, promoting policies and clinical programs to help aging adults live safe, healthy lives with strength, movement, and joy.

You will oversee staff, coordinate therapy programming, and ensure compliance with standards while building strong relationships with patients and facility personnel.

Qualifications

• Clinical background as a Certified Physical Therapy Assistant (PTA) or Physical Therapist (PT).
• Three to five years’ experience in long-term care (LTC) or senior living. Supervisory experience preferred but not required.
• Minimum degree from accredited college or university (PTA, PT).
• Licensed and registered (PTA or PT) in the state of assignment.

Responsibilities

• Create rapport and positive working relationships with facility staff and Legacy employees.
• Oversee internal marketing to build relationships with staff and residents; keep community leaders informed of Legacy programs.
• Manage site productivity and adjust to meet budget.
• Maintain a caseload to help the rehab department meet standards.
• Assess patient needs and develop clinical programs appropriate for facility census.
• Direct and train staff in clinical service delivery across disciplines.
• Coordinate daily rehab activities with facility staff to plan and implement therapy programming.
• Participate in clinical outcomes reporting and share with the customer.
• Manage staff schedules to meet caseload requirements and use scheduling tools.
• Ensure physician signatures on therapy plans (CliniFax) and related requirements.
• Foster team relationships and open communication among rehab staff, facility staff, and Legacy corporate personnel.
• Assist with staff orientation/training with guidance from ARD/MSM to meet standards.
