Job summary

Legacy Healthcare Services in Boca Raton, FL, seeks a Rehab Director to serve as the primary Legacy representative at the facility, promoting Legacy policies and clinical programs to help aging adults live safe, healthy lives full of strength, movement, and joy.

This role offers growth in leadership and clinical program development, with responsibilities including building relationships with facility staff and residents, overseeing productivity, and directing rehab service delivery across

Qualifications

• Three to five years’ experience in long-term care (LTC) or senior living.
• Minimum of a degree from accredited college or university (i.e., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in the state where assigned.

Responsibilities

• Create rapport and positive working relationships with facility personnel and Legacy employees.
• Oversee internal marketing efforts to build relationships with staff and residents.
• Manage site productivity and make changes to achieve budget.
• Maintain caseload to assist rehab department in meeting standards.
• Assess patient needs and develop clinical programs for the facility census.
• Direct and train others in clinical service delivery across disciplines.
• Coordinate rehab department daily with facility staff to plan therapy programming.
• Participate in clinical outcomes reporting.
• Manage staff schedules and use scheduling tools to meet caseloads.
• Handle CliniFax and physician signatures on therapy plans.
• Foster team communication between rehab staff, facility staff, and Legacy personnel.
• Assist with staff orientation/training to meet standards.

• Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
• Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
• Manage site productivity and makes necessary changes to achieve budget.
• Maintain a caseload that assists the rehab department in meeting standards.
• Able to assess patient needs and develop clinical programs appropriate for the facility census.
• Able to direct and train others in clinical service delivery across disciplines.
• Daily coordination of rehab department with facility staff to plan and implement therapy programming.
• Participates in clinical outcomes reporting and shares with the customer.
• Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
• Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
• Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
• Assists with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or

Speech Language Pathologies (SLP).

• Three to five years’ experience in long-term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
• Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.
