Job summary

Legacy Healthcare Services seeks a Rehab Director to lead the facility’s rehab department, promote Legacy clinical programs, and advance the health and wellbeing of aging residents.

You will build relationships with facility staff, oversee therapy scheduling and outcomes, develop clinical programs, and ensure compliance with state and federal standards while guiding a multidisciplinary team toward patient-centered care.

Qualifications

• Clinical licensure in the applicable state.
• 3–5 years of long-term care or senior living experience.
• Prior supervisory experience preferred but not required.
• Degree from an accredited college in PT, PTA, OT, COTA, or SLP.

Responsibilities

• Promote Legacy policies and clinical programs within the facility.
• Oversee rehab department operations and scheduling to meet caseloads.
• Develop and implement patient-centered clinical programs for the facility census.
• Mentor and train rehab staff across disciplines and coordinate with facility leadership.

• Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
• Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
• Manage site productivity and make necessary changes to achieve budget.
• Maintain a caseload that assists the rehab department in meeting standards.
• Able to assess patient needs and develop clinical programs appropriate for the facility census.
• Able to direct and train others in clinical service delivery across disciplines.
• Daily coordination of rehab department with facility staff to plan and implement therapy programming.
• Participates in clinical outcomes reporting and share with the customer.
• Manages staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
• Manages CliniFax and all requirements of physician signatures on the therapy plan of cares.
• Creates and maintains team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
• Assists with orientation/training for staff with guidance and assistance from ARD/MSM to meet company, state and federal standards.

Position requirements:

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathology (SLP).
• Three to five years’ experience in long-term care (LTC) or senior living. Some prior supervisory experience preferred but not required.
• Minimum of a degree from an accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in the state where assigned.
