Greenfield Rehab is seeking a Rehab Director—a licensed Physical Therapist or Physical Therapist Assistant—to lead therapy services in our skilled nursing facility in Galesville, WI. This role directs clinical and administrative operations across PT/OT/SLP, collaborates with nursing and interdisciplinary teams, and ensures high-quality, patient-centered care.

You will supervise staff, support career development, and maintain regulatory compliance while delivering direct patient care as needed.

Qualifications

• Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
• License as required by the State of Wisconsin to practice as a Physical Therapist.
• Graduate of an accredited school with an associate degree as a Physical Therapist Assistant.
• Certification or license as required by the State of Wisconsin to practice as a Physical Therapist Assistant.

Responsibilities

• Lead therapy services in the skilled nursing facility as Rehab Director.
• Provide clinical and administrative oversight of PT/OT/SLP services.
• Coordinate with nursing and interdisciplinary teams to ensure quality patient care.

This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency.
