You’ll drive clinical outcomes, mentor therapists, and partner with facility leadership while staying connected to patient‑centered care and quality of life for older adults. TMC supports growth, development, and work‑life balance in a therapist‑driven organization.

Qualifications

• Licensed PT/OT/SLP/COTA in the state of practice
• Graduate of an accredited PT/OT/SLP/PTA/COTA program
• 2+ years in healthcare; long-term care preferred
• Interest or experience in geriatric rehabilitation
• Passion for patient-centered care for older adults

Responsibilities

• Provide leadership of the facility's rehabilitation therapy program
• Lead, mentor, and support a multidisciplinary therapy team
• Deliver personalized, patient-centered rehabilitation focused on independence
• Partner with facility leadership, nursing, caregivers and interdisciplinary teams
• Coach and supervise clinicians with ongoing clinical guidance
• Oversee therapy operations including staffing, scheduling, documentation & regulatory compliance
• Identify opportunities to strengthen and grow rehabilitation services
• Monitor patient and program outcomes and drive continuous improvement
• Foster a culture of support, accountability and empowerment
• Serve as trusted partner to facility leadership and therapy team
