Job summary

Concord Post Acute is seeking a Director of Rehabilitation (DOR) in Concord, CA to lead the rehabilitation department and oversee PT, OT, SLP, and staff. You will mentor a multidisciplinary team, coordinate scheduling, ensure regulatory documentation, and develop programs to improve resident outcomes.

The role emphasizes clinical leadership, program development, and collaboration with hospital partners, physicians, and facility leadership.

Qualifications

• Licensed PT, OT, or SLP eligible to practice in California
• Bachelor’s degree in Physical Rehabilitation, Occupational Rehabilitation, or Speech-Language Pathology preferred
• Master’s or Doctorate degree preferred
• Previous rehabilitation leadership or supervisory experience strongly preferred
• Proficiency with Net Health and PointClickCare

Responsibilities

• Lead and supervise the rehabilitation department staff
• Coordinate scheduling and caseloads across PT, OT, SLP, assistants, and students
• Ensure regulatory compliance and accurate clinical documentation
• Develop and implement clinical programs based on resident and facility needs
• Monitor therapy quality, productivity, and outcomes
• Collaborate with facility leadership, nursing, physicians, and families
