Responsibilities

• Develop and implement outreach strategies to connect with local community organizations, physician practices, clinics, senior living facilities, and other healthcare providers.
• Build and maintain relationships with stakeholders to promote hospital services and referral pathways.
• Educate partners about hospital services, programs, resources available to patients.
• Collaborate with physicians, discharge planners, case managers, and other healthcare professionals to facilitate patient referrals to the hospital.
• Organize community events, health fairs, seminars, and workshops to promote the hospital's services.
• Collaborate with the hospital's marketing and public relations teams to develop targeted outreach materials.
• Assess healthcare needs and gaps in the community.
