Lifepoint Rehabilitation in Nacogdoches, TX is seeking a Clinical Liaison to educate the community about acute rehabilitation and develop census through face-to-face contacts with patients and referral sources. The role emphasizes building relationships with physicians, discharge planners, case managers, and social workers to align services with referral needs.

The position requires onsite and territorial travel, strong interpersonal skills, and a commitment to improving patient outcomes.

Qualifications

• Clinical degree and current license to practice as OT/PT/SLP/RN.
• Valid driver's license and reliable transportation.
• Experience collaborating with physicians, nursing management, discharge planners, case managers, and social workers.
• Health care setting experience preferred.
• Onsite and territory travel ~80%.

Responsibilities

• Educate the community on acute rehabilitation to develop a census through face-to-face contacts.
• Develop business based on the strategic goals of the rehabilitation program.
• Face-to-face connections within the territory to build relationships with referral sources to increase census.
• Identify barriers to the admission process and create solutions with the assistance of the program director.
• Requires onsite and in-territory work through face-to-face contact with patients, families, and referral sources.
• Completes in-person in-services and presentations to educate on acute rehabilitation programs and services.
• Other duties as assigned
