PAM Health Aiken is seeking a full-time Case Manager to coordinate care across disciplines for a designated patient population. You will review admission records, determine appropriate level of care, and develop discharge plans in collaboration with the team.

The role requires state licensure preferred, 3–5 years inpatient experience, and current BLS certification. Comprehensive benefits and a supportive environment are offered as the hospital opens this fall.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Interdisciplinary coordination with care teams is essential.
• 3–5 years inpatient experience preferred in acute/IRF/LTACH settings.
• BLS certification required.

Responsabilités

• Coordinate care for designated patient population and ensure appropriate admission decisions.
• Review inpatient records to optimize resource use and level of care.
• Assess discharge needs and plan referral/ Discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
