Skills And Experience

Minimum 2 years of experience required in a hospital work setting. Skills include:

• Colostomy care and management
• Continence skin care management
• Catheters management and self-care teaching
• Fecal incontinence program
• External catheters for both genders
• Pressure and venous ulcer management
• Skin and mucous membrane alterations
• Braden scale and pressure ulcer assessments
• Wound vac therapy and dressing changes
• G-tube/J-tube feeding and site care
• Infection control and isolation precautions
• Patient and family teaching
  
  

Patient Diagnosis

• Care of existing wounds
• Consultation on new wounds
• Ostomy teaching and support
• Moisture-associated dermatitis
• Pressure and shear injuries
  
  

Special Procedures/Unit Details

• Complete all consults within 24 hours (72 hours for weekend requests)
• Documentation in IVIEW, including staging and measurements
• Round and review needs for long-stay patients (7 days and beyond)
• Provide real-time educational opportunities to front-line staff
• Communicate potentially reportable wounds as appropriate
• Assist with dressings and initiate wound vacs with daily checks
• Perform new ostomy teaching
• Coordinate with bedside nurses and case management for discharge supplies
• Reassess high-risk patients every 2-3 days
