Summary:

Constellation Home Care is seeking a compassionate and skilled Registered Nurse (RN) Case Manager for a field-based role. The position involves driving to and treating patients in their homes, assisted living, and skilled nursing facilities within an assigned territory.

Responsibilities:

• Conduct head-to-toe assessments and provide skilled nursing care, including medication management and wound care, per physician’s plan.
• Visit patients for admissions, re-certifications, and discharges.
• Develop and coordinate the Plan of Care with healthcare professionals.
• Utilize the current OASIS assessment for patient evaluations.
• Educate patients and caregivers on disease management, medications, and treatment plans.
• Report daily to the Clinical Manager.
• Document care using an iPad within required timeframes.

Qualifications

Qualifications:

• Licensed Registered Nurse in the state of employment, graduate of an accredited RN program.
• Minimum of 1-2 years RN experience; home care experience preferred.
• OASIS experience is highly desirable.
• Experience with PleurX, Foley Catheters, Wound Vac, and Ostomy care is a plus.
• Reliable transportation and willingness to travel for patient care.
• Availability for some weekend and holiday shifts, including summer and winter holidays.
• Basic knowledge of iOS operating system.
