Job summary

Wellstone is seeking a Registered Nurse for the River Crossing Withdrawal Management facility in Alabama. The RN will monitor clients, document progress, and support the therapeutic team within treatment plans.

The role requires weekend shifts (Sat-Sun, 7:00AM–7:00PM) and compliance with state and federal regulations. Responsibilities include vital signs, discharge planning, supervising Behavioral Health Technicians, emergency response, and safe patient care.

Qualifications

• Active Alabama RN license.
• 2+ years in mental health or withdrawal management preferred.
• Excellent verbal and written communication skills.
• Proficient in Microsoft Office Suite.

Responsibilities

• Take and record vital signs and clinical data.
• Assist with discharge planning and documentation in EMR.
• Provide oversight to Behavioral Health Technicians.
• Administer injections as needed and document accordingly.
• Respond to crises and perform de-escalation when required.
• Coordinate with labs for test results and maintain timely documentation.
• Maintain safe, clean, and compliant facility environment.
• Attend required trainings and maintain licensure in Alabama.

Job description

Job Description: The Registered Nurse at our River Crossing Withdrawal Management facility will maintain client care by evaluating clients for emergent care needs and supporting our therapeutic team within client treatment plans. The RN will properly document the medical progress and notes of clients while adhering with Center, state, and federal regulations. This position will be working Saturdays and Sundays, 7:00AM - 7:00PM.What you’ll be doing:Conduct vital signs, i.e. orthostatic blood pressure, weight, height, pulse rate information on clients of our 16-bed facility.Assist with discharge planning.Provide oversight and guidance to the Behavioral Health Techs working with the clients.Obtain information from clients regarding his or her physical status.Make inquiries of clients to include any changes with him/her, symptoms, etc.Assess medication side effects and/or adverse reactions.Perform medication monitoring.Perform crisis intervention techniques as needed.Administer injections as needed and document in clients’ electronic medical record.Interface with local laboratories to obtain test results as needed.Complete and maintain documentation in a timely manner.Report any client or staff incidents or injuries to the appropriate supervisor in a timely manner.Ensure clients comply with rules, maintaining a clean and safe environment to include performance of housekeeping, assisting clients where necessary.Demonstrate good safety skills including fire drills, emergency procedures and back safety techniques.Promote a positive work environment, including positive relationships with clients, their families, supervisors, and co-workers.Attend staff meetings in-service meetings, and trainings as required; stay current on all trainings/certifications.Maintain Nursing licensure in Alabama.Provide a safe environment and promote quality care through adherence to established standards of care and facility policies, procedures and protocols.Report to work on time as scheduled, remaining on shift until able to be relieved; correctly complete and submit timesheets and leave requests in a timely manner.Report any facility or maintenance issues in a timely fashion and assist in the maintenance/housekeeping of the facility and equipment.Perform verbal and, at times, physical de-escalation techniques as required, in addition to crisis interventions.May be required to lift clients at times, with assistance.Complete other duties as assigned.This job description is only a summary of the typical functions of the job and is not designed to be an exhaustive or comprehensive list of all possible duties, tasks, or responsibilities that are required of the employee as they may change, or new ones may be assigned at any time with or without notice.What we’re looking for:Valid Alabama State LicenseMinimum of two years’ experience in the mental health fieldExperience in Withdrawal Management preferredExcellent verbal and written communication skillsProficient computer skills to include knowledge of Microsoft Office SuiteExcellent organizational and documentation skillsExcellent Customer Service experienceSAMA certification (or ability to obtain)
