Job summary

Affinity Hospice is seeking a registered nurse/case manager to plan, organize, direct, and provide hospice care with a focus on community health education. The role builds on community resources to serve individuals and families in home and community settings.

The position is full-time with weekend on-call responsibilities, offering strong benefits, sign-on bonus, and opportunities for advancement within a nationally recognized team.

Qualifications

• Graduate of an accredited nursing program (Diploma, ADN or BSN).
• Current State RN license and CPR certification.
• Minimum two years of RN experience in clinical care, home health or hospice.
• Valid driver's license with insured vehicle in good working order.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team.
• Develop and implement the patient plan of care.
• Deliver care with infection control measures (OSHA).
• Ensure documentation supports quality patient care.
• Monitor cases for third-party payer compliance.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call duties as assigned.

Position Summary:

The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nursebuilds fromthe resources of the community to plan and direct services to meet the needs ofindividualand families within their homes and communities.

Hours / Schedule:

Full Time; WeekendOnCall Position scheduled Friday 4:30pm - Monday 8:00am.

Minimum Qualifications:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse
• Current CPR certification
• Minimum of two years of experience as a Registered Nurse ina clinicalcare, home health or hospice setting preferred.
• Must be a licensed driver with an automobile that is insuredin accordance withstate and/or organization requirements and is in good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Able to lift, position and/or transfer patients.
• Able to lift supplies and equipment.

Key Responsibilities:

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care
• Provide careutilizinginfection control measures that protect both the staff and the patient (OSHA).
• Assure continuity of quality patient care delivered withappropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third-party payer.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Responsible for supervising hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals and initiatives.
