Job summary

Transitions Hospice is seeking an adaptable Registered Nurse to join our Extended Care Team (ECT) as a PRN visit nurse in the Bay City area. You will assess, treat, and support patients and families across the Great Lakes Bay Region with clinical precision.

This role offers flexible scheduling, travel within the service area, and the chance to make a meaningful impact when the regular team cannot be present. A current RN license and hospice experience are preferred.

Qualifications

• Must hold RN license in the state of employment.
• Graduate of an accredited nursing program.
• Hospice/home health experience preferred.
• Willingness to travel locally.
• Excellent communication and documentation skills.

Responsibilities

• Deliver nursing care across the full nursing process within your scope.
• Conduct patient and family assessments and contribute to care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from assessments and respond accordingly.
• Record timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and services are arranged and available for patients.
• Maintain and grow clinical skills through continuing education.

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
