Intermountain Healthcare seeks a Registered Nurse for a virtual role focusing on triage and transitional care management. Job Description:
This position functions as a vital member of the healthcare team who contributes to theThis position is a remote position. If you are currently an internal caregiver for Intermountain Health, you must reside in either CO or MT close to an Intermountain care site. If you are not a current caregiver for Intermountain Health and are applying externally, you must currently reside in Colorado or Montana near an Intermountain Health care site.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Performs clinical triage, including a comprehensive assessment, disposition and education using established triage protocols and guidelines
2. Manages transitions of care to ensure appropriate follow up care and mitigate risks associated with readmission.
3. Contacts patients following emergency department visits to ensure appropriate follow up
4. Uses registry data to identify gaps in services and initiates intervention
5. Refers patients to a variety of resources, including behavioral health, transportation services, nutrition, etc.
6. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
7. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
8. Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
9. Participates in multidisciplinary teams to improve patient care processes and outcomes.
10. Interacts with all staff, colleagues and team members in a professional and collegial manner
11. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards.
12. Performs other duties as assigned
