Intermountain Health is seeking a Clinical Program Coordinator in the Trauma department, located at Good Samaritan Hospital in Lafayette, Colorado. The role coordinates care from diagnosis through follow-up, guides patients through the care team, and provides education during hospitalization and after discharge.
Requires a Bachelor’s in Nursing, current RN license, BLS-HCP certification, and three years of RN experience. 40-hour week, with potential for on-call and weekends as needed.
The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$41.20 - $62.17
