Intermountain Health is seeking a Clinical Program Coordinator at Good Samaritan Hospital in Lafayette, CO. The role coordinates treatment processes from diagnosis through follow-up, guiding patients through collaboration with physicians, nurses, and ancillary staff, and providing education throughout the hospital stay.
The position requires a BSN, current RN license, BLS-HCP certification, and at least three years of RN experience.
The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$41.20 - $62.17
