Intermountain Health is seeking a Clinical Program Coordinator at Good Samaritan Hospital in Lafayette, CO. The role guides patients and families through the care team, coordinating treatment from diagnosis through follow-up and providing education during the hospital stay.
You will participate in rounds, review charts, analyze data for performance improvement, and help develop evidence-based policies while maintaining a QA program and ensuring regulatory alignment.
The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$41.20 - $62.17
