Job summary

Plan A Health is seeking a Registered Nurse to assess, manage, and provide comprehensive nursing care during a hospital stay, coordinating planning with multidisciplinary teams in a patient-centered approach.

You will perform admissions, transfers, post-op and discharge assessments; develop and evaluate care plans; administer medications; educate patients; monitor conditions; and contribute to safety, infection control, and quality improvement in a collaborative, respectful environment.

Qualifications

• Current RN licensure in applicable states with BLS certification.

Responsibilities

• Performs thorough nursing assessments at admission, transfer, post-op, and discharge.
• Develops and evaluates individualized care plans based on data and orders.
• Delivers nursing interventions, administers meds, manages IVs and referrals.
• Communicates with patients and families, plans discharge education.
• Monitors patient condition and responds to changes, including high-risk care.
• Follows safety, infection control, and quality standards; participates in QA.
• Delivers age-appropriate care to adults and geriatrics; fosters teamwork and mentorship.

Position Overview:

The Registered Nurse assesses, manages, and provides comprehensive nursing care during a patient's hospital stay, coordinating planning with multidisciplinary teams using a patient-centered approach.

Key Responsibilities

• Performs thorough nursing assessments, including admission, transfer, post-op, and discharge evaluations, documenting findings accurately.
• Develops, implements, and evaluates individualized care plans based on assessments, diagnoses, lab data, and physician orders, setting short and long-term goals.
• Provides and documents nursing interventions, collaborates with healthcare team members, administers medications, and manages IV fluids and referrals.
• Communicates effectively with patients, families, and staff, ensuring education, discharge planning, and respectful, compassionate service.
• Monitors patient condition, recognizes changes, and takes appropriate actions, including specialized care for high-risk patients and restraint/pain management.
• Maintains professional accountability, adheres to safety, infection control, and quality standards, and participates in performance improvement and evidence-based practices.
• Delivers age-appropriate care to adult and geriatric patients, utilizing effective communication and assessment skills.
• Fosters a collaborative team environment, mentors new staff, and ensures a respectful, supportive workplace culture.
