Job Summary

A Neonatal Advanced Transport Nurse Specialist (TNS) will be a highly functioning registered nurse with advanced education and training, who applies expert clinical practice, advanced procedural skills, and leadership to the inter-facility transport of both stable and critically ill neonatal patients. The Neonatal Transport Specialist is a Registered Nurse whose function is to provide clinical expertise in neonatal intensive care nursing and transport.

The Neonatal TNS has a strong foundation in the assessment and management of the critically ill neonate applying institutional practice guidelines and standards of care. The Neonatal TNS demonstrates an advanced level of medical and nursing knowledge, clinical and technical competence, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills among the care team as well as families, timely and compliant documentation in the medical record, and the review of performance toward providing an environment of safe quality care. In addition to providing patient care during transport, the Neonatal TNS is responsible for providing daily oversight for the effective and safe operation of transport equipment and supplies in conjunction with transport team members. The Neonatal TNS takes responsibility for managing professional development through ongoing participation in educational activities and programs, procedural simulation, as well as observation of clinical care in the neonatal intensive care unit all toward the establishment and maintenance of competency. This is performed in collaboration with transport leadership, respiratory care therapy, and the neonatal nursing management team members. The Neonatal TNS is responsible for the quality of standards of nursing care and transport care for the neonatal population. This role works collaboratively with the clinical and administrative teams to support and ensure high-quality standards of nursing care and professional practice to optimize patient, team, and system outcomes.

Essential Functions

  • Customer Service:

  • Consultation
    • Provides expert nursing consultation to clinical leadership staff for the resolution of nursing practice issues.
    • Collaborates with the Critical Care Transport leadership, NICU Medical Director(s) and nursing management team.
  • Expert Clinical Practice
    • Utilizes specialized knowledge and nursing expertise to identify and analyze complex needs of children and families and provides and/or directs delivery of expert nursing care to these children and families.
    • Demonstrates advanced skill in the delivery of direct patient care following clinical practice guidelines for neonatal patients to ensure safe interfacility transport
    • Applies advanced procedural skills to care for the neonatal patient through airway management including supraglottic airway placement, endotracheal intubation and mechanical ventilation, thoracentesis, thoracotomy, intraosseous line placement, and umbilical line placement.
    • Participates with interdisciplinary team in the formulation, evaluation and revision of clinical practice guidelines for neonatal transport and intensive care patient population.
    • Implements new techniques or methods of patient care delivery that enhances patient care by improved outcome.
  • Care Coordination: (may include, but not limited to)
    • Implementing clinical practice guidelines and standards of care for transport
    • Develops and evaluates clinical practice guidelines with consideration of current standards of care and best available evidence
    • In communication with, and under the direction of, the medical control physician (MCP) and collaboration with other transport team members insures the continued evaluation and management of the patient during transport
  • Clinical Support and Education:
    • Participates in educational needs assessments for patients/families and healthcare providers.
    • Collaborates with the BCH Critical Care Transport Educator as well NICU Nurse Educator and other members of the neonatal intensive care unit to develop, implement, and evaluate formal and informal education programs for nurses and other healthcare providers. The educator position has not been hired yet.
    • Collaborates with patient nurses and providers to assess and provide education for patients/families with complex learning needs.
    • Develops educational resources for staff.
    • Provides resource materials and educational presentations to other departments and the professional and lay communities as needed. Evaluates teaching materials on an annual basis and coordinates revisions as appropriate.
    • Demonstrates that educational activities have a positive impact on patient care outcome and/or clinical nursing practice.
  • Communication and Collaboration:
    • Maintains ongoing communication and collaboration with interdisciplinary health care team including neonatology.
    • Facilitates and communicates with patients, family, and staff to promote continuity of care throughout the clinical course.
    • Participates in hospital-wide committees and improvement projects as appropriate, representing neonatal intensive care expertise with special considerations for interfacility transport.
  • Documentation:
    • Documents key components of the patient¿s history and presentation, assessment, as well as clinical management during the transport process through the medical record where applicable.
    • Documentation is timely, meets hospital compliance standards and captures patient acuity, diagnoses and management.
  • Quality Management:
    • Participates in Multidisciplinary Quality Improvement Committee meetings as available or as relevant to transport-specific issues.
    • Ensures the staff and NICU programs are compliant with all relevant federal, state, local regulatory standards, including The Joint Commission.
    • Maintains a safe work environment and ensures that all events are reported, investigated, and resolved, completing transport debrief summaries per Critical Care Transport Program Standards
    • Is proactive in evaluating the work environment to identify potential unsafe conditions and implements a plan of action to correct issues, as indicated.
    • Maintains a constant state of regulatory compliance preparedness though ongoing assessment and action.
    • Participates in the quality improvement process by formulating, implementing and evaluating transport nursing standards of care, integrating research findings into clinical practice, and facilitating the conduct of research in area of expertise.
    • Identifies patient safety issues, works with appropriate staff/departments to implement methods to reduce risk of patient injury, and participates in development of emergency policies and or preparedness activities.
    • Participates in departments/specialty interdisciplinary QI applying CQI principles and is active in developing QI monitoring based on clinical practice guidelines.
    • Reviews patient events for Critical Care Transport Program as well as the NICU and provides guidance and education to support staff identifying and implementing opportunities for improvement in patient care.
  • Professional Practice and Development:
    • Seeks opportunities for active engagement in research and the analysis of evidence-based practice.
    • Actively participates in Critical Care Transport Program staff meetings and clinical case series, APN council meetings, evidence-based practice meetings
    • Maintains ongoing education and membership in professional organizations.
    • Demonstrates Professional Practice behaviors including preceptor/mentoring, education and instruction of students, nursing staff, nurses, graduate and novice nurse practitioners.
    • Identifies personal and professional strengths and weaknesses, develops pertinent goals and objectives, engages in activities to enhance effectiveness, and evaluates practice about establishing goals.
    • Develops annual professional goal plan and is current with all required education and certifications.
  • Research/Clinical Development:
    • Implements research findings and recent clinical developments in specialty area into current clinical practice.
    • Assists other members of the health care team in interpreting current research and integrating into practice those findings relevant to the care of neonates and infants.
    • Provides consultation for product trials, QI activities, and conducts product evaluation trials, providing recommendations for capital and minor equipment/supply purchases.
  • Leadership:
    • Combines expert knowledge and skills in consultation with nursing and other departments to develop systems that ensure delivery of quality care. Contributes to the professional growth of nursing practice through the mentoring of staff.
    • Forms partnerships with healthcare providers to plan, implement and evaluate transport care.
    • Participates in the development and implementation of division /unit and specialty specific standards.
    • Participates in goal development for neonatal transport program; assist in defining resources (personnel, educational, systems development, supplies/equipment) required to implement new programs and evaluates program¿s impact.
    • Serves as a mentor in the development of all levels of nursing and transport staff
    • Represents hospital and/or specialty area at local/regional meetings as requested.
Other Related Duties

  • Performs other related duties as assigned.
  • When not engaged in transport or transport related activities, Transport Nurse Specialists are encouraged to assist in the NICU with patient care.
  • Willingness to train and participate in fixed-wing and helicopter transports in the future.

  • Keeps other members of the leadership team apprised of the program¿s activities.
  • Communicates with the MCP during the transport process, providing an initial assessment and care plan and further providing necessary updates on changes in clinical status.

  • Self-directed clinician who practices under BCH Oakland NICU and Critical Care Transport Program guidelines as written and follows evidence-based practice.

  • Recognizes the importance for communication with the MCP and seeks assistance for decisions in optimizing the patient care plan for transport.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $84.78 - $109.85 (Hourly Rate).

Required Qualifications

  • 3-years of Pediatric Transport Nurse Specialist experience
  • 2-years of Critical Care nursing experience in Pediatrics (neonatal, pediatric or cardiac intensive care, or pediatric emergency department experience)
  • Required to have the flexibility to orient and work at all BCH locations
  • Sit: Up to 10 hours per day
  • Stand/Walk: 8-10 hours per day
  • Bend/Stoop: Up to 4 hours per day
  • Reach: Up to 6 hours per day
  • Repetitive Use of Upper Extremities: Up to 10 hours per day
  • Lift/Push/Pull : 25 pounds, over 25 pounds with assistance or equipment
  • This job requires the ability to hear alarms clients and/or instruction. The ability to see accurately from 2 inches to 20-ft. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential function of this position.

Preferred Qualifications

  • 2-years of Neonatal Intensive Care nursing experience
  • Current specialty certification in one or more of the following: CFRN, CCRN, CTRN, RNC-NIC, C-NPT, CPEN
  • Bachelor of Science in Nursing (BSN) preferred.

License/Certification

  • Active California Registered Nurse license.
  • Current American Heart Association CPR certification (BLS)
  • Current American Academy of Pediatrics Neonatal Resuscitation Program Advanced Provider (NRP).
  • Current Pediatric Advanced Life Support Certification (PALS) (preferred on hire or within 3 months)
  • S.T.A.B.L.E. Certification (preferred on hire or within 6 months of hire)
