Qualifications

• RN license active in state of employment.
• Minimum 2 years of nursing experience in skilled nursing, rehab, long-term care, or hospital settings.
• Prior supervisory or charge nurse experience required.

Responsibilities

• Lead and coordinate a team of RNs, LPNs, and CNAs to deliver high-quality care.
• Ensure care plans are followed and residents receive personalized attention.
• Monitor resident health and provide clinical guidance for interventions.
• Conduct rounds to evaluate resident well-being, staff performance, and compliance.
• Oversee medication administration and maintain accurate clinical documentation.
• Assist with admissions, transfers, and discharges for smooth transitions.
• Serve as a clinical resource and mentor to promote staff development and best practices.
• Collaborate with physicians, therapists, and interdisciplinary teams to optimize outcomes.
• Respond promptly to emergencies and support staff in critical situations.
