Position Summary

We are seeking a compassionate, detail-oriented, and independentRegistered Nurse (RN)to join our home care agency on aper-case basisas aClinical Supervisor. This flexible, field-based position offers the opportunity to play a foundational role in shaping our agency’s quality of care and operational success.

The position has the potential to transition into afull-time clinical leadership roleas our client base grows.

The RN will conductin-home client evaluations, develop individualized service plans,train and assess caregivers, and performmonthly follow-up evaluationsfor active clients. Ideal candidates are experienced home care nurses who value clinical autonomy, flexible scheduling, and meaningful patient impact.

Compensation

• Client Conversion Bonus:$50 per case that transitions to an ongoing client
• Monthly Re-Evaluation Visit:$70 per visit
• Caregiver Training & Skills Evaluation:$45 per hour
• Compensation is per completed service; no guaranteed minimum hours

Key Responsibilities

• Develop and maintainindividualized service and care plans
• Performmonthly supervisory visitsto review care, monitor health outcomes, and ensure care plan adherence
• Provideinitial and ongoing trainingfor Home Health Aides (HHAs), including condition-specific competencies
• Document all client visits, trainings, and evaluations in accordance with agency and regulatory requirements
• Serve as a clinical liaison to caregivers and families, addressing questions or changes in condition
• Ensure all services comply withNew Jersey state home care regulations
• Contribute to agency development, operational processes, and community outreach during the launch phase

Qualifications

• ActiveRegistered Nurse (RN) licensein the State of New Jersey (Required)
• Bachelor of Science in Nursing (BSN)preferred
• Minimum3 years of experience in home care or community-based nursing(Required)
• Knowledge ofNew Jersey home care standardsand documentation protocols
• At least21 years of age
• Strong clinical assessment and judgment
• Clear verbal and written communication skills
• Comfortable providing caregiver education and clinical feedback
• Organized, detail-oriented, and able to manage multiple cases
• Proficient in Microsoft Office and digital charting/documentation systems
• Self-motivated with the ability to work independently

Physical Requirements

• Able to travel to client homes and agency office as needed
• Able to sit, stand, and use a computer for extended periods
• Able to lift up to 15 pounds occasionally

As the agency scales, this role may expand into afull-time supervisory or clinical director position. You will have the opportunity to grow with us and help define the care standards, training protocols, and service delivery model of a mission-driven, community-based agency.
