Qualifications

Registered Nurse Supervisor for our new Marion PACE (Program of All Inclusive Care for the Elderly) Center opening to serve our Neighbors in McDowell County!

Job Summary and Qualifications

Role Summary:

Under the supervision of the Operations Manager, manages the daily operations of the PACE Center. Responsibilities include the planning, coordination and implementation of all operations in the PACE Center. Works in conjunction with the Social Workers to assure that concerns and support are followed up on to families, participants, and caregivers. Works in conjunction with the Therapy team (PT/OT/ST/RT) to assure that recreational activities and wellness classes are provided as scheduled. Works in conjunction with the Registered Dietician to deliver an excellent dining experience and assure that diets are provided as ordered. Works with the Triage RN to ensure that Program Aides complete assigned daily tasks and report areas of concern. At the Alternative Care Site, Supervisor serves as the Health Care Coordinator for ADHS.

Essential Accountabilities :

Collaborates and consults with interdisciplinary team to achieve desired outcomes.

Performance Criteria:

1. Demonstrates leadership through conflict resolution and service recovery with participants, families, and staff by following recommended guidelines.
2. Supports the IDT decisions and coordination of 24 hour care delivery.
3. Leads morning huddle for day center staff to disseminate information and reinforce information.

Maintains regular attendance at and participate in interdisciplinary team meetings and careplans as the regulatory representative to actively communicates relevant issues to the team, ensures all team members are informed and up-to-date, participates in the development of the plan of care and monitors and articulates participants' progress and decline.

Assists in the coordination of management activities and serves as support to Operations Manager with daily activities in the day center

Performance Criteria:

1. Responsible for hiring and training the day center team
2. Assure necessary staff are up to date with CPR and First Aid training.
3. Provide/Coordinate personal care training and/or training to assist participants with eating.
4. Working Supervisor that is competencies to provides care when needed to support staff
5. Recommends changes in service delivery and staffing as required by the development and growth of the PACE.
6. Developments PCT schedule, oversight and completion of daily-required documentation and reports associated with State and Federal agencies.
7. Conducts and documents coaching of staff, monitoring progress of staff
8. Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
9. Maintain the PACE Center, its supplies and its equipment as required by plant operations, and state and federal agencies
10. Coordination of delivery of home delivered meals, DME, supplies and miscellaneous items as requested by the interdisciplinary team.
11. Monitors operations to ensure consistent compliance with established day center PAR levels.

Ensures the provision of, or directly provides needed direct participant care, evaluates outcome, consults with other specialist and IDT as required.

Preformance Criteria:

1. Provides Administration on-call coverage on a rotation basis.
2. Documents accurately and in a timely manner in EMR.
3. Delegates to other licensed staff and unlicensed staff according to scope of practice.
4. Delivers care in organized manner based on acuity and schedule per scope of practice.
5. Identifies problems and implements solutions to meet needs of participants and families.
6. Implements care planning interventions and ensure compliance in the day center.

Supports performance improvement activities for Adult Day Services.

1. Works with Quality dept to support Annual State Adult Day Recertification process.
2. Coordinate Lean events for Adult Day Services (Mapping, Kaizen, 5S) as needed.
3. Create/Monitor action plans as needed.

Ensure standard work is occurring among all programs.

Determine ways to measure/track quality outcomes and ensure completion as well as necessary follow up for improvements .

Making certain that health and personal care services as outlined in IV.B., page 19 of the North Carolina Standards for Certification for Adult Day Health Services, are provided to participants consistent with participants' service plans

Provides continuing education for staff, participants, and families, and/or community.

Performance Criteria:

1. Educating other staff members to emergency procedures and providing information to staff and caregivers about health concerns and conditions of participants
2. Precepts new program aides and PCT(s)
3. Facilitates competency validation per scope of practice
4. Initiates individualized teaching plan for participants and families, communicates, documents and reinforces plan.

Responsible for consistent program needs, with the North Carolina Department of Health and Human Services Health Care Coordinator requirements, and with the Nursing Practice Act include:

Performance Criteria:

1. Learning and applying the rules and regulations for the health care component of Adult/Health Care services as outlined by current Adult Day Health/Care Standards of the North Carolina Division of Aging and Adult Services
2. Implementing the health care components of the established service plan which include: medication monitoring and administration; wound care enteral or parenteral feedings bowel

or bladder training and maintenance programs tracheostomy care and suctioning delegating nursing care duties to qualified unlicensed personnel

1. Monitoring participants' responses to medical treatment plan and nursing interventions and revising plans of care as necessary
2. Reporting and recording results of nursing assessments, care rendered, and participants' response to care to the IDT

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 15 days of employment start date
• First Aid Basic must be obtained within 30 days of employment start date
