Job summary

Gateway Home Health Corp. in San Joaquin County, California, is seeking a Home Health Registered Nurse (RN) to provide patient-centered care and case management for home health patients.

The position offers an hourly rate of $72–$80 and coverage area across San Joaquin County; applicable benefits are available for eligible staff. The ideal candidate holds a California RN license, CPR/BLS, and has wound care experience.

Qualifications

• Valid RN license in California.
• BSN preferred.
• Current CPR/BLS certification.
• Valid California driver's license.
• Minimum 1 year experience with wound care.
• Strong knowledge of medical terminology, physiology, and diagnostic evaluation.
• Experience in case management or managed care is desirable.
• Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals.
• Ability to work independently and make sound clinical judgments.
• Proficient computer skills for documentation and data management.
• Experience with Synergy EMR is preferred but not required.
• Understanding of the PDGM Medicare model.

Responsibilities

• Provide patient-centered care and case management to individuals in need, and work one on one with patient/family to improve health and stability.
• Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans.
• Collaborate with healthcare professionals to coordinate and facilitate patient care and goals.
• Monitor and evaluate patients' progress and adjust care plans as necessary.
• Ensure accurate and timely documentation of patient information and medical records including clinical notes, orders, and charting within the Medicare required 48-hour window.
• Advocate for patients' needs and rights within the healthcare system.
• Educate patients and their families on disease management, treatment options, and available resources.

• Current CPR/ BLS Certification

• Experience documenting accurate OASIS data is preferred but not required
• Understanding of the PDGM Medicare model
