Job summary

St. John's Community Services in Washington, DC is seeking a Registered Nurse for full-time health services delivery.

You will supervise health needs of adult participants, dispense medications, perform nursing assessments, and maintain comprehensive health records in accordance with Medicaid regulations. The role requires a DC RN license, a Bachelor’s degree in Nursing, and at least three years of nursing experience, with the ability to work within a coordinated care team and adhere to safety

Qualifications

• Requires a Bachelor’s Degree in Nursing and an active DC RN license.
• Three years of nursing experience is required for this role.
• Background check and health certificate as per funding jurisdiction.

Responsibilities

• Provide health services to participants following standard nursing practices.
• Dispense medications as prescribed by physicians during program hours.
• Complete nursing assessments including record review and interviews.
• Perform routine health screenings (BP, weight, etc.).
• Provide emergency care and coordinate with the Nursing/Medical Care Coordinator.
• Maintain health and immunization records and update as needed.
• Prepare monthly progress notes and quarterly summaries.
• Interpret health history and health needs for Care Plans.
• Offer health counseling and education to staff, participants and families.

Job Summary

To provide health services to participants, in accordance with standard nursing practices. These procedures should be consistent with the mission and philosophy of St. John’s Community Services and adhere to all Medicaid regulations. This includes direct provision of health services to meet the individual needs of each client in a manner that is respectful of their rights and dignity.

Essential Functions

• Ensure health supervision of adult clients in the State's programs, including:
• Dispense physicians’ prescribed medications during program hours as directed by physicians’ orders, following standard nursing practices
• Ensure completion of nursing assessments including record review, significant family member/provider interview, developmental history and current physical status as assigned by program’s RN.
• Perform routine screening for blood pressure, weight, and other specific concerns as noted by consumer’s program
• Provide emergency care.
• Work with Nursing & Medical Care Coordinator for health deficit identification
• Maintenance of health records and immunization records, notifying significant family member/provider when updating is needed.
• Ensure completion of progress notes monthly with quarterly summaries as assigned.
• Interpret special diets and nutritional requirements of all adults for the consumers and staff.
• Ensure preparation of health history and interpret special health needs as part of the Individualized Support Plan or Care Plan program for each participant.
• Provide health counseling and/or education for staff, participants, and/or significant family members or caretakers, in collaboration with Nursing & Medical Care Coordinator, with regard to interpretation of health information, guidance and counseling regarding health behavior, and recommendations regarding individual health conditions.
• Perform other job related duties as assigned

QualificationsRequired Knowledge, Skills, and Abilities

• Requires a Bachelor's Degree and R.N. with three years of experience; experience working with individuals with intellectual/developmental disabilities preferred.
• Current license as a Registered Nurse in the District of Columbia is required.
• Must meet SJCS and funding jurisdiction current requirements for criminal background/records check.
• Valid health certificate to meet current funding jurisdiction requirements;
• Valid state-issued driver’s license with current driving record in good standing required.

Work environment involves everyday risks or discomforts which require normal safety precautions typical of such places as offices, e.g., use of safe work place practices with office equipment, and/or avoidance of trips and falls.

Home environment involves everyday risks or conditions that require normal safety precautions such as stairs, rugs, appliances etc. and requires the avoidance of trips and falls. Additionally, medical work with individuals involves knowledge of medical protocols, exposure to human behavior, and other safety precautions. Proper precautions must be taken when assessing and/or interacting with individuals served with regard to lifting, transferring, turning, positioning, administering medications, etc.
