Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to provide home health visits in Allentown and surrounding communities. You will follow care plans, administer medications, perform wound care and assessments, and coordinate with clinical teams to ensure quality patient care.

We value independence, reliable communication, and strong EMR skills. Pennsylvania RN licensure, 1+ year of clinical experience, and willingness to travel are required.

Qualifications

• Current RN license in Pennsylvania is required.
• Minimum of 1 year of recent clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and computer literacy with EMR.
• Ability to travel to cases as assigned.

Responsibilities

• Follow a designated care plan based on patient needs.
• Make home visits to clients in designated geographic areas around Allentown.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical/Client Services Manager.
• Follow up on doctors' orders and document actions.
• Perform client assessments as needed and coordinate case management.

Registered Nurse (RN) Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions; reporting changes to Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors’ orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a Registered Nurse (RN)

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
