Job summary

Salyersville Nursing and Rehabilitation Center in Kentucky is seeking an LVN/LPN to oversee the total nursing care of residents on the assigned unit. You will administer medications, transcribe orders, and monitor patient status while coordinating with CNAs and therapists.

The role requires a current KY LVN/LPN license and at least 2 years of related experience with good standing in the state registry. This on-site position emphasizes safety, documentation, and teamwork.

Qualifications

Must hold a current LVN/LPN license in Kentucky.
Minimum of 2 years related nursing experience.
Good standing in the state nursing registry.
Strong charting and communication skills.

Responsibilities

Responsible during the shift for total nursing care of residents in the assigned unit.
Administers prescribed medications and treatments per nursing techniques.
Transcribes physician orders and documents care accurately.
Conducts frequent rounds and evaluates care delivery effectiveness.
Maintains safety and comfort; responds to emergencies per policies.
Collaborates with CNA and staff to implement care plans.
Monitors vital signs and notifies changes to supervisor/physician.
Supports care conferences and contributes to care planning.
Ensures documentation reflects the care plan and resident response.
Participates in in-services and staff meetings; performs additional duties.
