Registered Nurse (RN) Responsibilities:

• Perform home health visits, admissions, assessments, and follow individualized care plans
• Administer medications, wound care, and treatments as prescribed
• Monitor patient conditions and communicate changes to the clinical team
• Coordinate care and ensure accurate, timely documentation
• Utilize electronic medical records (EMR) on a state-of-the-art tablet
