Job summary

Avera Health is seeking aRegistered Nurse for Avera at Home Aberdeen to provide comprehensive nursing care to home health patients. You will follow ANA standards, collect patient data, develop and implement care plans, and collaborate with families and clinicians to achieve optimal outcomes.

Responsibilities include data collection, diagnosis-focused planning, and ongoing evaluation of patient progress, with emphasis on safety and quality.

Work where you matter.A Brief OverviewResponsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.What you will doCollects patient health data.Analyzes the assessment data in determining nursing diagnoses/problem focus.Identifies expected outcomes/goals individualized to the patient.Develops a plan of care that prescribes interventions to attain expected outcomes.Implements the interventions identified in the plan of care.Evaluates the patient’s progress toward attainment of the outcomesSystemically evaluates the quality and effectiveness of nursing practice.Collaborates with the patient, significant others, and healthcare providers in providing patient care.Utilizes research/evidence-based findings in practice if appropriate.Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.Essential QualificationsThe individual must be able to work the hours specified.
