Job summary

Massachusetts Health & Hospital Association in Missouri seeks a Registered Nurse (RN) in the Transitional Care Unit to provide direct patient care in a fast-paced, multidisciplinary setting.

You will assess, diagnose, plan, implement, and evaluate care, educate patients and families on disease management, medication, and post‑discharge needs, promote safety and quality, and collaborate with a diverse team to achieve optimal outcomes.

Qualifications

• Nursing diploma or associate degree required.
• RN license essential.
• Bachelor’s degree preferred with <2 years of experience.

Job description

Role: Registered Nurse (RN) in the Transitional Care Unit, providing direct patient care in a fast‑paced, multidisciplinary environment.

• Responsibilities: Assess, diagnose, plan, implement, and evaluate patient care; develop and communicate individualized care plans; educate patients and families on disease management, medication, and post‑discharge needs; promote patient safety, quality, and regulatory compliance; collaborate with healthcare professionals to ensure optimal outcomes; evaluate physical, psychosocial, and emotional needs, considering patient preferences and barriers.
• Work Environment: Care for diverse patients with complex medical conditions at a Level I Trauma Center; gain experience in a dynamic setting that fosters clinical growth and prepares for specialty nursing roles, especially in emergency care.

Qualifications & Skills: Nursing diploma or associate degree required; RN license essential; bachelor’s degree and <2 years of experience preferred. Critical thinking, assessment, communication, and teamwork skills are vital. Ability to work safely and maintain alertness is mandatory.
