Job summary

Home Health Care, Inc. in St. Paul, MN is seeking an RN Visit Nurse to provide skilled nursing visits throughout St. Paul and surrounding communities.

You will assess patients in their homes, deliver care, educate families, and document your findings. This full-time role offers independence, on-call and weekend rotation, and strong clinical leadership. A Minnesota RN license and home health experience are preferred.

What Your Visits Could Include

• Comprehensive RN assessments
• Skilled home health nursing visits
• Wound assessment and wound care
• Medication reconciliation and management
• Injections and other skilled nursing procedures
• Monitoring acute and chronic conditions
• Patient and caregiver education
• Identifying changes in condition
• Communicating clinical concerns with physicians
• Coordinating with PT, OT, SLP, social work, and other clinicians
• Supporting safe transitions home following hospitalization or rehabilitation
• Helping prevent avoidable emergency department visits and rehospitalizations
• Completing timely documentation in Homecare Homebase (HCHB)

Your Job Isn't Just to Complete the Visit.

A great Visit Nurse looks beyond the immediate task. Why isn't the wound improving? Is the patient actually taking the medication correctly? Does the caregiver understand the discharge instructions? Who We're Looking For

We're looking for an RN who can be trusted with both the clinical responsibility and independence that come with field nursing.

You'll Need

• An active Minnesota Registered Nurse (RN) license
• Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
• Strong clinical assessment and critical-thinking skills
• Confidence working independently
• Excellent patient and caregiver communication
• Strong organization and time-management abilities
• Ability to recognize and appropriately escalated changes in condition
• Reliable transportation and willingness to travel throughout the St. Paul service area

Home health experience is certainly valuable.
