What Could Your Day Look Like?

You'll travel throughout the St. Paul area, completing skilled nursing visits that may include:

• Comprehensive RN assessments
• Skilled home health nursing visits
• Wound care and wound assessment
• Medication reconciliation and management
• Injections and other skilled nursing procedures
• Monitoring acute and chronic medical conditions
• Patient and caregiver education
• Assessing progress following hospitalization or rehabilitation
• Identifying and communicating changes in condition
• Coordinating with physicians and other healthcare providers
• Collaborating with PT, OT, SLP, social work, and interdisciplinary team members
• Helping prevent avoidable emergency department visits and rehospitalizations
• Completing timely documentation in Homecare Homebase (HCHB)

Who We're Looking For

We're looking for an RN who likes a little movement in their day. Someone who enjoys meeting different people. Someone who can walk into a new environment, assess what's happening, and confidently determine what needs to happen next.

You'll need:

• An active Minnesota Registered Nurse (RN) license
• Previous nursing experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
• Strong assessment and critical-thinking abilities
• Confidence working independently in the field
• Excellent patient and caregiver communication skills
• Strong organization and time-management abilities
• Ability to manage a daily schedule of patient visits
• Reliable transportation and willingness to travel throughout the St. Paul service area
• Previous home health experience is valuable, but experienced nurses from other clinical settings are encouraged to connect with us.
