Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will triage, educate patients, coordinate care, and support transitions of care to improve outcomes and patient experience.

You will collaborate with providers and care partners, manage referrals and authorizations, and help patients access community resources while promoting preventive care and chronic disease management.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.
• Minimum 1–2 years of clinical experience in a medical or physician office preferred.

Description: Make a Meaningful Impact Beyond the Bedside! Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Registered Nurse (RN) to join our outpatient care team. In this role, you'll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.
