Signature HealthCARE is seeking Registered Nurses to join our Hermitage, Hillcrest, and Riverside locations. You will provide compassionate nursing care, support residents and families, and collaborate with the healthcare team to deliver quality care.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication; ability to work independently and exercise sound clinical judgment.
• Experience with electronic health records (EHR).
• Proficient with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician/party of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
