Job summary

Connections Health Solutions is seeking a Registered Nurse (RN) to assess individuals in crisis, document triage and health information, and provide education on behavioral health and treatments. The role emphasizes crisis intervention within a physician-led, data-driven team.

You will perform nursing assessments, participate in multidisciplinary care planning, and support de-escalation and safety procedures in a fast-paced environment.

Qualifications

• Graduated from an accredited Nursing program.
• Unrestricted active RN license.
• Able to perform Safe Clinch patient restraint techniques.
• American Heart Association BLS certification.
• Competent in ASAM 3.7 alcohol/drug focused nursing assessment.
• Vaccination status per company policy with documentation.
• Must have Virginia Fingerprint Clearance.
• Must pass VA DBHDS background check.
• Must pass VA DSS Central Registry search.
• Physical requirements to lift up to 50 lbs and perform safe interventions.

Responsibilities

• Conduct thorough Nursing Assessments including Triage, Medical/Behavioral Health assessments and med administration.
• Collaborate with Multidisciplinary Clinical team to plan treatment.
• Educate patients and families on psychiatric diagnosis, treatment options, and health literacy.
• Use verbal interventions to de-escalate agitation or unsafe behavior.
• Document seclusion/restraint considerations per standards and debrief when applicable.
• Transcribe orders accurately and manage medication reconciliation.
• Discharge planning and coordination with other facilities as needed.
• Perform additional duties as assigned.

• +$2/hr -> Monday-Thursday Nights
• +$6/hr -> Friday-Sunday Nights

What You'll Do:

We are hiring a Registered Nurse (RN) who assess individuals and documents pertinent information including Triage, Behavioral Health, Physical Health, and/or Substance Abuse issues. Demonstrates knowledge of Crisis Intervention and the ability to de-escalate potentially aggressive situations and will provide education to consumers and their families regarding Behavioral Health diagnosis, medications, and treatments.

• Completes detailed Nursing Assessments with individuals and documents pertinent information including Triage, Medical/Behavioral Health assessments and medication administration utilizing appropriate assessment tools. Accurately record all medication brought in by individual on medication reconciliation/storage form.
• Plans individual treatment as part of the Multidisciplinary Clinical team.
• Provides education concerning Psychiatric diagnosis, treatment options, psychopharmacology, and overall health literacy to individuals, families and all others involved in the individual’s care.
• Intervenes with appropriate verbal interventions to diffuse out of control or potentially dangerous behaviors. Demonstrates ability to effectively use therapeutic option theory and techniques and can describe and demonstrate their roles and responsibilities.
• Demonstrate a thorough knowledge of Seclusion/Restraint standards, guidelines, techniques and initiate documentation and debriefing with individuals for any seclusion/restraint incidents as required.
• Accurately transcribes Provider orders and administers medications as prescribed. Attempts to have individual sign medication consent form. Performs chart checks and signs off as a second noted on medications prescribed. Draw labs and provide wound care as ordered by Providers when required.
• Complete nurse to nurse at discharge or when accepting individual from another facility.
• Performs all other duties as assigned.

What You'll Bring:

• Graduated from an accredited Nursing program
• Unrestricted active license as Registered Professional Nurse through State Board of Nursing or hold a multistate licensure privilege
• Must be able to perform Safe Clinch patient restraint techniques
• American Heart Association BLS
• Working on ASAM 3.7, the RN shall have competencies and experience in conducting an alcohol or other drug focused nursing assessment
• The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
• State of Virginia Fingerprint Clearance
• Must be able to pass Virginia Department of Behavioral Health & Developmental Services (VA DBHDS) background check
• Must be able to pass Virginia Department of Social Services (VA DSS) Office of Background Investigations - Central Registry Search

  • Safely perform physical interventions (e.g., standing, pivoting, reaching, kneeling, lowering to the floor, and supporting another person’s weight)
  • Lift, push, pull, or carry up to 50 lbs. occasionally
  • Tolerate prolonged walking, standing, and observation in a fast-paced environment
  • Work with individuals who may exhibit agitation, aggression, or unpredictable behavior

It would be great if you had:

• Bachelor’s degree in Nursing
• At least 1 year of psychiatric, behavioral health, or relevant experience
• Previous emergency room or crisis psychiatric care experience
• Experience working with individuals with addiction and addiction management
