The Registered Nurse (RN) assumes primary leadership accountability and responsibility for milieu management and directing patient care activities of other nursing staff. The RN maintains standards of professional nursing practice in accordance with facility policies, external governing and credentialing bodies, performance improvement standards, and psychiatric nursing standards.

The RN assesses patients using the nursing process to develop individualized care plans, implements interventions, and evaluates outcomes to promote, maintain, and restore physical and mental health while preventing illness. The RN interprets age/disability-specific information relevant to the client population served and ensures the safe delivery of medication according to policies, procedures, and physician orders. Accurate reporting and documentation of symptoms, responses, and progress are essential responsibilities. The RN also maintains professional development, continuing education, and licensure in compliance with regulations and standards.

Essential Functions

1. Utilize the nursing process—assessment, planning, intervention, implementation, and evaluation—to develop individualized care and treatment plans.
2. Identify risk levels, precautions, and expected outcomes, including discharge planning; prescribe interventions to achieve expected outcomes; evaluate intervention effectiveness; conduct ongoing assessments from admission to discharge; and initiate crisis interventions as needed.
3. Demonstrate knowledge of psychiatric disorders and implement appropriate nursing interventions tailored to patient needs, promoting wellness and maintenance of mental health.
4. Administer medications safely, educate patients on their medications, and recognize signs of adverse reactions.
5. Maintain professionalism, accountability, and responsibility for decision-making and quality of care, staying informed on psychiatric nursing practices.
6. Review, revise, and document medical records accurately within facility guidelines; transcribe and implement physician's orders; and record patient responses and progress thoroughly and concisely.
7. Follow up on patient care responsibilities, communicate effectively with staff to ensure continuity of care, and assist as needed.
8. Delegate duties appropriately, demonstrate a positive attitude, and advocate for patient needs and safety.
9. Use nursing knowledge and resources to meet the needs of the facility, patient population, and staff, enhancing evidence-based practices.
10. Float among clinical services as qualified and competent, and perform other duties as assigned.
