Job Title: Registered Nurse (RN) Job Code: HCP071/Registered Nurse 2

Shift: PRN/average of 16 hours per week (Thursday & Friday Shifts)

Job Hours: 9:00am-5:00pm

Base Location: May Vary

Division/Department/Program: Assertive Community Treatment (ACT) Program


The Assertive Community Treatment (ACT) team provides intensive intervention, treatment, rehabilitation, and support services to individuals with severe, persistent, and acute mental illnesses or addictive diseases. Responsibilities include participating in client-centered comprehensive assessments of psychiatric history, mental status, diagnosis, physical health, education, employment, and social development. You will also participate in the development of treatment and discharge plans based on observations of individual strengths, needs, abilities, and preferences. This role involves providing effective follow-up and referral services that facilitate individual linkages, communication, and continuity of services, as well as facilitating individual-family interactions and keeping supervisors and treatment team members informed of significant developments. You will complete comprehensive nursing assessments and provide links to medical care.


Under general supervision, you will learn and assess client health problems and needs, develop and implement nursing care plans, and maintain medical records. This role includes administering nursing care to ill, injured, convalescent, or disabled clients, and may involve advising clients on health maintenance and disease prevention or providing case management.


Duties & Responsibilities:
  1. Administers prescribed medications, noting times and amounts on clients' charts.
  2. Consults and coordinates with health care team members to assess, plan, implement, and evaluate client care plans.
  3. Maintains accurate, detailed reports and client records.
  4. Modifies client treatment plans as indicated by clients' responses and conditions.
  5. Monitors all aspects of client care, including diet and physical activity.
  6. Records and reports symptoms and changes in clients' conditions.
  7. Observes nurses and visits clients to ensure proper nursing care is provided.
  8. Provides education to clients and family.
  9. Provides health care, first aid, immunizations, and assistance in convalescence and rehabilitation.
  10. Receives additional training, as required, to gain full proficiency and experience in all areas.
  11. Other duties as assigned.

Minimum Qualifications:

Bachelor's degree in nursing from an accredited college or university OR associate degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.


Preferred Qualifications:

Experience working with the Severe and Persistently Mentally ill population, ACT teams, or individuals identified as frequent users of ERs, Psychiatric Hospitals, Crisis Stabilization Units, and the criminal justice system. Nursing experience. Community experience working with individuals with SPMI. Experience working in a high-stress, fast-paced environment with a very emotionally and physically changing population. Experience working in multi-disciplinary teams. Ability to multi-task and manage time.
