Job summary

Signature HealthCARE in Roanoke Rapids, NC is seeking registered nurses to deliver high-quality patient care in a team-focused environment. You will provide direct nursing care, administer medications, assist with ADLs, and supervise nursing staff on shift.

The role emphasizes patient dignity, safety, and collaboration with an interdisciplinary team to develop individualized care plans and maintain thorough documentation per facility policies.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor and observe residents’ status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by Director of Nursing.

How You Will Make a Difference

What You Need To Make a Difference

• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
