Job summary

You will triage telephone calls, coordinate care with providers, and support families while maintaining confidentiality. Travel to regional offices is required as needed; current CT RN license and CPR certification are mandatory.

Qualifications

• Current CT RN license and CPR certification required.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and formulates nursing plan of care; ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention in prompt, precise, and professional manner.
• Assists patients and family members with concern and confidentiality.
