Job description

Maxim Healthcare Services is seeking a Transitional Care Team (TCT)-Registered Nurse (RN) to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Transitional Care Team (TCT)- Registered Nurse (RN) will ensure smooth transitions for our PICU and NICU patients from the hospital to the home. They will provide education and training for parents and families, as well as facilitate orientation for new caregivers.

Responsibilities
  1. Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  2. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
  3. Collect, analyze, and interpret data from healthcare team members and document actual or potential nursing diagnoses.
  4. Document the patient’s plan of care, including nursing diagnoses, expected outcomes, and interventions.
  5. Perform interventions based on priorities, plan of care, hospital policies, and care standards.
  6. Revise the plan of care as needed based on evaluations and changes in the patient's condition.
  7. Use clinical judgment to establish priorities and meet patient care needs.
  8. Perform other duties as assigned.
Requirements
  • Current RN license for the practicing state.
  • Current health certificate (per facility or state regulation).
  • Current PPD or Chest X-Ray.
  • Experience in PICU and NICU settings.
  • Hospital setting experience.
  • Flexible scheduling.
  • Current BLS certification.
  • At least one year of RN experience.
