Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment to coordinate patient care and delegate activities of the patient care team.

Job Responsibilities

  1. Assesses the patient's physiologic health status
  • Conducts patient interviews, explains policies and procedures, reviews patient charts, and answers questions courteously;
  • Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health;
  • Determines mobility, sensory deficits, prostheses use, skin condition, and adjusts care plans accordingly;
  • Assesses pain levels and management;
  • Documents assessment findings and care plans.
  • Assesses the patient's psychosocial health status
    • Gathers perceptions of care and expectations;
    • Determines coping mechanisms, knowledge, and comprehension abilities;
    • Identifies cultural and ethnic needs to reduce anxiety and improve satisfaction;
    • Documents psychological status and care plans, including support services.
  • Identifies patient outcomes
    • Develops outcome measurement criteria;
    • Identifies actual and potential problems;
    • Assesses teaching needs based on psychosocial and developmental factors;
    • Establishes individualized patient goals and outcome statements.
  • Formulates and prioritizes the care plan
    • Identifies necessary care activities and priorities;
    • Coordinates resource use to achieve outcomes;
    • Documents and collaborates on the care plan with the healthcare team.
  • Implements the care plan
    • Ensures safety precautions are followed;
    • Applies professional skills and revises care as needed;
    • Administers treatments safely and correctly;
    • Uses equipment properly;
    • Provides emotional support;
    • Performs procedures using scientific principles;
    • Records observations and responses accurately;
    • Maintains proper documentation;
    • Acts effectively in emergencies;
    • Creates a restful environment and communicates with families;
    • Participates in discharge planning and referrals.
  • Evaluates patient outcomes
    • Measures and documents care effectiveness;
    • Performs variance analysis for quality improvement;
    • Implements systems to improve care quality;
    • Maintains accurate treatment and condition records.
  • Collaborates with the healthcare team
    • Works with physicians on care plans;
    • Implements treatment regimens accurately;
    • Assists during treatments and diagnostics;
    • Reports patient status changes;
    • Delegates and supervises care activities appropriately.
  • Performs grade I-IV decubitus care
    • Prevents skin breakdown;
    • Applies dressings and compresses;
    • Performs irrigations and related procedures.
  • Administers medications safely
    • Identifies medications and understands their effects;
    • Prepares and administers medications per policy;
    • Educates patients on medication use and interactions.
  • Participates in patient and family education
    • Explains conditions and care;
    • Records and communicates assessment data;
    • Utilizes teaching resources;
    • Plans for discharge and post-hospital care;
    • Evaluates teaching effectiveness.
  • Performs other duties as required.
  • *ADA Essential Functions

    Job Qualifications

    • Graduate of an accredited School of Nursing; Bachelor's degree preferred. Enrolled in an accredited program within 24 months if hired after September 1, 2010, with a goal to obtain a BSN within five years.
    • Current NY State RN license.
    • BCLS, ACLS preferred.
