Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment to coordinate patient care and delegate activities within the patient care team.

Job Responsibilities
  1. Conducts patient interviews, explains policies and procedures, reviews patient charts, and assesses health status across various systems. Adjusts care plans accordingly, assesses pain levels, and documents physiologic and psychosocial health status. Communicates patient perceptions and care expectations.
  2. Identifies coping mechanisms, knowledge levels, cultural/ethnic needs, and provides appropriate psychological support and education. Develops outcome criteria and identifies patient problems and teaching needs based on assessments.
  3. Develops patient outcome statements and goals, prioritizes care activities, and coordinates the use of supplies and medications. Implements and documents the care plan, monitors patient safety, and revises care as needed. Provides emotional support and participates in discharge planning and referrals.
  4. Administers treatments and medications safely, observes and records patient responses, and maintains accurate documentation. Acts effectively during emergencies, ensuring a calm environment and effective communication with the patient and family.
  5. Evaluates the effectiveness of care, participates in performance improvement initiatives, and collaborates with the care team. Provides direction to team members and assists physicians during treatments and procedures.
  6. Interprets and implements treatment regimens as prescribed, and keeps the care team informed of patient status. Performs skin care procedures, administers medications, and educates patients and families about their health and medications.
  7. Performs related duties as required, adhering to all applicable laws and standards.
Job Qualifications
  • Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing preferred. Enrolled in an accredited BSN program within two years, with a goal to obtain a BSN within five years of employment.
  • Current RN license in New York State and necessary certifications.
