Job summary

AdventHealth in Celebration, FL is seeking a Registered Nurse to coordinate patient care within Oncology-supportive programs. You will liaise with the multidisciplinary team, manage documentation, and ensure timely care with physicians and services to achieve optimal outcomes.

The role requires RN licensure, BLS, and 1+ years of professional nursing experience, preferably in oncology. Orthogonal duties include patient education and collaborative treatment planning.

Job Description:

• Acts as a liaison across the multidisciplinary team to ensure proper orders, medical records, consents, and other required documentation are received.
• Coordinates timely care with physicians and supportive services departments.
• Maintains professional relationships with customers, patients, and team members.
• Provides direct patient care coordination within the Community Care and Transitional Care Programs.
• Other duties as assigned.
• Educates patients, families, caregivers, and community providers to safely perform care and achieve health goals.
• Evaluates the effectiveness of care plans and monitors patient and family responses to achieve desired outcomes.
• Identifies performance improvement initiatives and assists in designing or implementing programs to address needed changes.
• Participates in community education and outreach programs to promote health and wellness.
• Collaborates with Imaging Coordinator, RN Navigator, and referring providers to navigate patients through treatment planning, medical management, and surveillance.
• Serves as an educational resource for patients and families on treatment planning and medical management.
• Facilitates communication between healthcare team, patients, other departments, and outside entities by answering phone calls, responding to emails, and managing messages in the EMR and patient portal.

Knowledge, Skills, and Abilities:

• Ability to provide age-appropriate cancer education. [Required]

Education:

• Bachelor's [Preferred]

Field of Study:

• Graduate of an accredited school of nursing
• in nursing

Work Experience:

• 1+ professional nursing experience [Required]
• 1+ professional nursing experience in oncology [Preferred]

Licenses and Certifications:

• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Oncology Certified Nurse (OCN) [Preferred]
