Position Overview
The On-Call Registered Nurse organizes and directs hospice care, emphasizing community health education and experience. The nurse directs immediate services to meet the needs of individuals and families in their homes and communities, covering nights and weekends.
Essential Job Functions
- Respond to calls from Triage during assigned shifts, making visits to patients with changing conditions and completing weekend admissions.
- Conduct initial and ongoing assessments of patients and families to determine hospice needs for after-hours calls, including physical assessments and medical histories.
- Provide professional nursing care utilizing all elements of the nursing process.
- Assess and evaluate patient status, develop and revise care plans, and use health data to determine nursing diagnoses.
- Administer medications and treatments as prescribed, and counsel patients and families on nursing and related needs.
- Provide health care instructions, assist with activities of daily living, and facilitate self-sufficiency and comfort care.
- Handle after-hours on-call responsibilities, remaining available for patient care calls and visits as needed.
- Document patient conditions and care provided accurately, communicate with physicians, and coordinate with community health personnel.
- Teach self-care techniques, ensure a safe environment, and support continuity of care with family and interdisciplinary team.
- Attend team meetings, supervise ancillary personnel, and pursue professional development.

Qualifications
- Graduate of an accredited nursing program
- 1-2 years recent acute care and home care experience
- Current nursing license and CPR certification
- Good communication and nursing skills, ability to lift and move patients, and valid driver’s license with insured vehicle
