Job Description

As a Peritoneal Dialysis Registered Nurse (PD RN), you will provide education on peritoneal dialysis at Stamford at Home Dialysis Center. You will educate patients and their care partners in a clinical setting, enabling them to dialyze from home. You will oversee the ongoing care of each patient to ensure their treatment remains on track.

Schedule: Flexible Monday-Friday 8am-5pm. No Weekends, No Holidays! Coverage in one facility with support for others as needed.

Training: Excellent paid training provided! Dialysis experience is not required; nephrology nursing offers a new set of skills for newcomers.

Responsibilities:

1. Provide comprehensive patient care: Assess, diagnose, and implement care plans, manage cases, and observe the impact of your care.
2. Empower patients: Teach self-administration of dialysis and promote health independence. Support the growth of the home dialysis program and educate new in-center patients.
3. Lead and support: Work independently, build relationships with patients, support systems, and care partners.

Schedule Details:

• Typically, an 8-hour day, Monday through Friday, with weekends and holidays off.
• Manage your own cases and schedule flexibly.
• Home visits and on-call shifts vary and are paid accordingly.

Preferred Qualifications:

• Experience in dialysis or healthcare education.
• Case management experience.
• Familiarity with telehealth technologies.
• Certifications such as CNN, CDN, CPDN.
