Job summary

Liberty Home Care is seeking a Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor in Niagara County. This part-time role offers a 25-hour workweek, Monday through Friday, 9am to 2pm, with benefits.

Responsibilities include patient assessment, care planning, ongoing reassessment, documentation, and supervision of PCA/HHA staff. Must have NYS RN license, an associate degree, and at least one year of nursing experience; willingness to travel within Niagara County.

Qualifications

• NYS Licensed Registered Professional Nurse is required.
• Associate's Degree in Nursing is required.
• One year of nursing experience is required.
• Ability to travel throughout Niagara County for home visits.
• Home care experience is a plus.

Responsibilities

• Assess newly referred patients and determine home care needs.
• Create patient-centered plan of care with the patient.
• Reassess needs and discuss changes with physician/family.
• Prepare documentation of patient progress for services continuation.
• Provide supervision and training of PCA/HHA staff.

Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! This Part Time opportunity offers great benefits and a consistent M-F 9a-2p - 25 hour per week schedule. Schedule: 9am - 2pm Monday - Friday (25 hours per week) Service Area: Niagara County

• Provide assessment of newly referred patients and assess health needs in home care setting
• Work with patient to create a patient centered plan of care
• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
• Provide supervision and training of PCA/HHA staff
