Job summary

Professional Case Management is hiring Registered Nurses to deliver in-home healthcare, building one-on-one patient relationships and improving health outcomes in patients' homes.

You control your schedule and can pursue education, travel, volunteering, or family time while providing essential care to those who served our country.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted RN license in the state(s) of practice.
• Prefer minimum of one year experience as an RN in acute care or will consider new graduates.
• Current CPR certification and TB test.

Responsibilities

• Provide in-home nursing care to patients.
• Maintain safety and a supportive home environment for clients.
• Adhere to agency policies and procedures and coordinate with care teams.
