Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced RN's to provide patient-centered home health care in Houston, TX and surrounding areas. The role emphasizes one-on-one visits, flexible scheduling, and delivering evidence-based care in patients' homes and communities.

You will perform assessments, administer medications, educate families, and collaborate with an interdisciplinary team to achieve durable, functional outcomes for patients across the recovery spectrum.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate, or BSN) or vocational nursing.
• Current RN license in the state.
• CPR certification current.
• Compassionate, friendly demeanor and strong communication.
• Valid driver\'s license and reliable transportation.
• Ability to travel locally and flexible schedule.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments of physical, psychosocial, and environmental needs
• Assess conditions, chart observations, and perform vital signs and medication review
• Administer medication as prescribed by physician
• Educate patients and families on home health care strategies and devices
• Provide effective communication with patients, families, team members, and other health professionals

Areas Of Responsibility Include But Not Limited To

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients' conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient’s physician
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing, or vocational nursing
• Current state license as an RN
• Proof of current CPR certification
• Compassionate, friendly demeanor
• Excellent communication skills
• Current driver's license and reliable transportation
• Ability to work a flexible schedule and travel locally
