Job summary

IHA in Hartford, CT seeks an Oncology Nurse Navigator (RN) to guide cancer patients through their care journey. You will coordinate plans, educate patients, and advocate for timely treatment in a fast-paced setting.

Responsibilities include coordinating appointments, monitoring progress in Epic EMR, and linking patients with support services and resources. A BSN is preferred and CT RN license is required; CPR certification expected.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current Connecticut RN license in good standing.
• Current CPR certification (or ability to obtain).
• Experience in oncology, hematology, infusion, navigation, or care coordination preferred.
• Strong clinical assessment, patient education, communication, and organizational skills.
• Proficiency with electronic medical records; experience with Epic preferred.
• Ability to thrive in a fast-paced environment while providing compassionate patient-centered care.

• Develop partnerships with community organizations and patient support groups to enhance patient access to care.
