Job summary

Advocate Health is seeking an RN for the Hospital at Home Program in Park Ridge, IL. The role is full-time with a 12-hour day shift and rotating weekends, providing hospital-level care to patients in their homes.

The position offers a sign-on bonus of $5,000 and requires an Illinois RN license, BLS, and relevant acute care experience. Travel within the Lutheran General Hospital service area is expected.

Qualifications

• Active IL RN license required; BLS certification.
• Valid driver’s license with clean record.
• 2+ years acute inpatient care or ED experience preferred.
• Strong IV, phlebotomy and clinical assessment skills.
• Ability to work independently in home settings; tech-savvy.

Responsibilities

• Deliver acute, hospital-level care in patients’ homes.
• Perform skilled clinical tasks (IV starts, phlebotomy, meds).
• Monitor patient conditions and escalate when needed.
• Collaborate with a virtual and in-person interdisciplinary team.
• Use evidence-based practice to ensure safe, patient-centered care.
• Coordinate discharge planning and ongoing care needs.
• Promote safety and high-quality outcomes.

What You'll Do

What We're Looking For

• Active RN license in the state of Illinois
• BLS certification (AHA)
• Valid driver’s license with a clean driving record
• 2+ years of recent acute inpatient care or ED experience preferred
• Strong IV, phlebotomy, and clinical assessment skills
• Ability to work independently in home settings
• Excellent communication and critical thinking
• Comfortable with technology and remote monitoring tools
• Work Environment Travel within designated service area
• Provide care in private home settings
• Physical activity required (lifting, bending, standing, etc.)
• Exposure to clinical and environmental variables typical in-home care
